How to Apply for a Home Care Package
Simple Steps
Step 1: Check Your Eligibility
Aged 65+ (or 50+ for Aboriginal/Torres Strait Islander people)
✅ Noticing changes in your ability to manage daily tasks (e.g., cleaning, cooking, mobility)
✅ Want to stay independent at home longer

Step 2: Get an Assessment
📞 Contact My Aged Care to request a free Aged Care Assessment (ACAT/ACAS).
🏠 Meet with an assessor (in your home or via phone) to discuss your needs.
📝 Receive your approval and package level (Level 1–4, based on care needs).
Step 3: Choose Prime Care & Share as Your Provider
